 Thanks for your ticket.  Please:
  * identify Open Firmware version used by your upgrade to 12.1.0,
  * confirm Open Firmware version used when the problem happened was Q4D17,
  * confirm Open Firmware version now installed is Q4D21,
  * check if downgrading Open Firmware hides the problem,
  * send the output of p2 with the drive attached before power up,
  * send the output of p2 with the drive attached after ok prompt,
  * send the output of {{{" /" ['] (show-usb2) scan-subtree}}} with the
 drive attached before power up.
