 I think I found a way to reproduce this with 656 and my rescue-action key:

 1) download [http://download.laptop.org/xo-1/os/official/656/jffs2/
 os656.{img,crc}] and [http://dev.laptop.org/~mstone/rescue-action.tar.gz
 rescue-action.tar.gz].

 2) move both into the root of a USB key and untar the action key.

 3) on an unlocked machine (a B4 in my case), copy-nand 656 and type
 'boot'. The action key will boot, will fail to mount part of the
 versions/run/656/sys, and will powerdown.

 4) remove the USB key and boot. The exception described above will be
 raised.
