 On occasions the libertas firmware will enter what we've called "frozen
 mode".  In this mode, all MLME commands misbehave.  Scan will return a
 large table of zeroes, and Ad-Hoc Start will timeout.

 It is hard to enter into this mode, but once in there is no way out other
 than reloading the usb8xxx module (which reloads the firmware).

 One way we've been able to cause the problem is by issuing ad-hoc start
 commands (iwconfig msh0 mode ad- essid moo) while NetworkManager was
 running.
