Whatever it was, I had the same symptoms (on my
never-as-far-as-I-know-locked B4).  I worked around it by starting the
OFW prompt and manually telling the machine to "boot u:\boot\OLPC.FTH"
which then completed the upgrade process.  Your mileage may vary.
