 the attachments fail1.log and fail2.log show slightly different sequences
 leading up to the error.  in both cases, a single preceding successful
 resume is shown as well.

 it seems odd to me (but may be normal) that hub resume is happening at the
 same time as, and apparently asynchronously to, port resume.
