 Tried to reproduce this with various external USB devices.

 Messages like "pxau2o-ehci pxau2o-ehci.0: detected XactErr len 0/8 retry
 #" can be generally caused with some USB devices by slowly wiggling their
 connectors back & forth and slowly removing said connector from the
 XO-1.75.

 The "usb 1-1.2: usb_disable_device nuking all URBs" message and disabling
 of all USB ports could only be reached by inserting & removing a USB 1.1
 lowspeed device.  Otherwise removing all USB devices just puts the USB hub
 to sleep.  If we are not already in this state, high-speed USB 2.0 devices
 can be inserted & removed repeatedly without any issue.

 Need to do some further analysis.
