 I kept out the USB key when powering on; and inserted it as soon as OFW
 started loading from NAND.  So before the kernel ever got control, the USB
 key was there.  It's always easier to assume user error rather than a
 coding bug -- for the programmer, anyway.

 I examined boot/olpc.fth.  It's a straightforward programming error.  Any
 time the SPI flash gets written, the lease is never created.  The system
 powers off in that circumstance, before returning from update-firmware to
 update-both to update-activate-and-boot.  It's a bug in the EC, preventing
 us from restarting the EC cleanly after rewriting its firmware.  (Mitch
 and Richard know about it.)

 If you're going to write a lease file and reprogram the SPI flash, you
 have to write the lease
 file first, before losing control of the system.
