 I experienced the same problem ~1-2 months ago. Sequence of actions was:
 1. Write my stuff to the USB flash drive on PC with Windows, Safe to
 remove, unplugged from the port.
 2. Plugged into OLPC, turned it on, pressed ESC to get into OFW shell,
 then "dir u:\" - showed me an error.
 3. Unplugged usb stick, plugged into PC - it is recognized as a removeable
 drive in Windows, but it can't be read, written, formatted, partitioned,
 nothing.

 USB drive is 'imation', 1GB, IEBF471772, was bought 1 day before usage in
 OLPC, and was used purely for testing with OLPC laptop.

 Mechanically contacts look not damaged, USB port in OLPC works good too.

 The OLPC usb port to which I was connecting the usb stick was the one
 right behind the videocamera.
