Comment:

 Fixed by svn 749.

 The problem was a side effect of an optimization to avoid wasting memory
 storing superseded dirents.  If the first dirent in block n+1 had an
 earlier version than the the last dirent in block n, but the same name and
 pino, the earlier one would incorrectly replace the later one.  The fix
 was to stop the optimization at block boundaries.
