 Under certain error conditions the USB stack on the device side does not
 return a handshake packet. We have added code to better handle those error
 conditions.  With each fix we've provided a private build of the firmware
 to all those working on the problem.

 We have seen the frequency of the failures gradually drop, which indicates
 that there may not be a single root cause but several independent problems
 with the same observable symptoms.  At the time of this writing several
 teams (cjb, cozybit, Marvell-Pune) are trying to reproduce this problem on
 the latest debug build.  Only cjb has been able to reproduce the issue but
 not reliably.
