 What Mark refers to as 'failure mode 1' looks like this:

 When we try using the normal driver it also has failures but it's more
 complicated -- it keeps failing to load the firmware and resetting the
 device in an endless loop.  It seemed easier to debug using the firmware
 flash tool rather than the driver.
