Comment:

 The "doesn't boot at all" problem is OFW, but the "goes into the GUI for
 one second" problem is probably not.

 I think the root cause of the OFW issue is that multi-card readers have
 multiple logical unit numbers (LUNs) for the different slots.  The
 automatic "disk" device detection code always tries LUN 0, and if the card
 is plugged into a slot that uses a different LUN, OFW's boot code won't
 find it.  In many cases, the following workaround will work:
    ok devalias  disk  /usb at f,4/disk at 1    \ or @2 or @3, depending on the
 card slot
    ok boot-configure
    ok boot
