New description:

 With an ext3 USB disk instead of ext2 as the instructions require ...

 The first OFW activation detected the attached USB disk and booted the OFW
 from it.  The second OFW activation hung on an autoreinstallation, the
 message on the display was:

 /pci/usb at f,5/scsi at 0,0
 /pci/usb at f,5/scsi at 0,0/disk
