[Olpc-uk] UK Repair Centre XO survey

As part of the UK Repair Centre's ramp-up plan[1], I'd like to do a
survey of XO ownership in the UK to better understand the Repair
Centre's target market.

Right now I have the simplest thing I could think of that might
possibly work:
