[Localization] Message to my fellow pt_PT translator

Hi, to my unknown (to me) fellow pt_PT translator, I've gone through
your translation and fixed some terms to match the ones used in the UI
(for example, you had Home=Início, while in the UI I had more literaly
translated Home=Casa ).

P.S.- Is there a way to see the origin of commits in Pootle, or who is
the other pt_PT translator? So that we could talk directly?
