 Cause of second try duplicate names resolved:
  * uses of $create-file need $delete in front of them so that a duplicate
 file is not created.  ext2 driver allows duplicate files,
  * $create-file did create the new file, then immediately close the file
 and call $open-file to open the ''original'' file,
  * fix for duplicate names in svn 3662.
