Comment(by greenfeld):

 This happened again on the same SKU 296 system as [comment:4 above],
 except the system never resumed doing suspend/resume cycles, and powering
 on the hub again via the EC results in more failure to enumerate errors.

 I toggled the hub on/off repetitively and ended up with the same (new)
 result.
