 Note that this is incredibly easy to reproduce with debian wheezy w/ stock
 3.2.x kernel when the root fs is ext4.  I've done it many times on
 multiple 1.5 machines.  Any time the machine is rebooted/power is cut, or
 the kernel crashes (which is unfortunately quite often), this bug occurs.
