Comment:

 I think I've exhausted my options for diagnosing this.  The cost of a new
 USB drive is way lower than my cost of continuing, and there's no
 indication at the moment that this problem is systemic.

 There remains a possibility that your XO-1.75 is faulty, or that the USB
 drive is faulty in a way that is only evident with OpenFirmware on
 XO-1.75, but not with Linux.

 Your photograph shows the XO-1.75 is a B1.  Have you reproduced this
 problem with a C1 or C2?  If you haven't got one, we can send you one.
