Comment(by dsd):

 Diagnosed by simulating command failures on a random basis. Found two
 issues related to command handling while resetting the card due to
 communications problems.

 Fixed in x86-3.3 c079dc74e6f330e27e5877cc14d64b42e15ecf69 and
 2b06ed644149d0546340f3b8e11a993492a0736f

 Sam, please test this kernel as it comes out of the autobuilder, before we
 add to build. Thanks.
