 Sorry Martin, I wasn't aware of your first patch, as trac sends no mail
 for attachments.  Please be sure to add a separate note.

 Reviewed the second patch.  It looks okay, but it would require another
 few test cycles to prove it.

 Richard, I thought I heard you say this was already happening in
 production, and the workaround was to repeat the runin.  Is that correct?

 Samuel, can we have some detail on the frequency of this problem?  Like
 how many complete runs have been made, and how many were affected by this
 problem?  I've never seen it, so I don't have any gut feel.
