 Thanks! My analysis is similar:

  * During startup, only one process is triggering rwtab mounts, and is
 clearly before (and not concurrent with) runin. There should be no race
 condition/
  * During shutdown, only /etc/init.d/halt unmounts, and is clearly after
 hwclock. hwclock doesn't seem for fork into background. The possibility
 remains that halt is called twice accidentally and races with itself --
 there is no flock to protect from this.
  * The affected unit I saw repro'd after 150 reboots, a random ramp unit
 went through 4400 reboots without hitting this. I want to lay my hands on
 an affected unit again :-)
