Comment(by pgf):

 is the invalid mapping ever observed at the OFW level?  or only in linux?
 being able to make it happen at OFW would go a long way to isolating it.

 if you can reproduce again, please "stop olpc-kbdshim-udev", and see if
 bad mapping remains.
