 After os41 is released, migrate the responsibility of providing
 /boot/olpc.fth out of bootfw and into olpc-os-builder.

  * remove /boot/olpc.fth from firmware build environment,
  * cease generating a bootfw package with each OpenFirmware release,
  * start generating an olpc-firmware package,
  * change olpc-os-builder to use olpc-firmware package, and add
 /boot/olpc.fth as a module script,
  * place olpc-firmware build scripts within version control.
