Comment:

 Unfortunately I cannot say what causes disassociations, as I have seen
 them too.  But usually I see in dmesg that the AP has asked us to
 disassociate us on XO-1.5's & 1.75's.  (Alternatively someone is forging
 this, or the card for some reason thinks it has been asked to.)

 I do not however recall seeing going from associating->disconnected
 directly, although our local DHCP server failing to provide an IP address
 has been a source of many early disconnects.

 Since I do not see an extreme case of this happening suggesting a major
 flaw in firmware, I suppose this can be closed until someone can come up
 with some way to reproduce unexpected disconnects or unless we hear of
 them happening a lot in the field.
