Comment(by dsd):

 Sam found that the kernel that retries the commands fails in the same way,
 the retried commands are also ignored by the device.

 I'm now working on resetting the card when this happens. A sub-optimal
 solution but the best we can do in the face of firmware/hardware bug right
 now.
