 Running Q5A01ma or Q5a02mb on an XO-3 motherboard, we sometimes see that
 OFW doesn't properly work with a USB key inserted in the USB A port.

 The symptoms are that after a boot, an attempt to reflash OFW fails
 silently:

 Reading u:\q5a02.rom
 Got firmware version: CL3   Q5A02  Q5A
 Checking integrity ...
 ok
 }}}

 A dir u: following this silent abort will properly list the contents of
 the USB disk.   fs-update shows the same behavior.
 Typing p2 sees the USB disk, but does not fix the problem.

 This happens randomly.   It is not triggered by having the USB key present
 at OFW boot, or by it not being present at boot.

 After a reboot of OFW, these operations frequently work properly again.
 Richard has seen the same behavior, on a different motherboard.
