 much of the confusion here comes from an omission in XO-1's rfkill
 implementation.  on XO-1, the rfkill state, as set by the network control
 panel, and recorded by the .rfkill_block_wifi flag file, is not honored at
 boot time.  so the wlan will always be enabled on a fresh boot.

 the attached patch will fix this specific problem, and should fix (i
 think) the first two issues noted in the description.
