 I missed 2 lines when reading this before. The device did indeed go away:

 16 minutes after the failed scan and automatic reset, the device goes away
 without explanation. As if it was disabled from within sugar (but bernie
 says this is not happening).

 Next step is to enable libertas debug messages so that we have more
 information on whats happening at the point when the device runs off.
