 Reproduced by loading the laptop with five instances of:

 {{{
 dd </dev/zero > /dev/null
 }}}

 and clicking rapidly on the Terminal icon in the activity ring.

 Additional observations about the affected state:
  * frame shows a Journal icon, then a static Terminal icon, then a pulsing
 Terminal icon,
  * clicking on the Journal icon, then the static Terminal icon results in
 the highlight moving without user direction to the pulsing Terminal icon
 ... that is one instance of Terminal cannot be selected by the user,
  * using Alt/Tab rotates focus between the Journal icon and one one of the
 Terminal icons, the other one cannot be reached by Alt/Tab,
  * the process list shows two Python processes running Terminal with
 identical command line arguments; normally two Terminal instances have
 quite different command line arguments, especially the -a flag value,

 Stopping the active Terminal instance using Control/D leaves a Terminal
 instance described by the Frame but it is unselectable.  The Terminal
 activity Python process remains present on the system.
