Comment(by mikus):

 I have an unmodified B2 machine (SHC9370111D) that ran fine for months and
 months, but can now act up (segment violation?) during OFW. The bypass
 that allows me to continue using this machine is to completely reset the
 hardware (pull battery and AC), or enter 'bye' at the ok prompt, before
 depending upon OFW to not segment fault.

 It's been several months since test-all has been able to complete on this
 machine. The faulting problem calmed down during the time I was using
 q3a46 (IIRC), but now again occurs randomly. q3a48 test-all (before
 faulting) tells me the machine fails the heat spreader test.
