 I think this behavior is specific to the Delta Electronics model ADP-15 PH
 A adapters.  They have the round case, and the plug does not have a molded
 cable clip on the strain relief.  The squarish case adapters model
 P018WA120J do not have the problem.  The Delta ADP17FB A adapters also
 seem to work (round case, strain relief does have molded cable clip).

 The symptom is that the "bad" adapters tend to "trip" when connected to an
 XO1.5.  When they "trip", the output voltage drops down to 2.5V (from the
 nominal 12).  Once they have tripped, you often have to unplug and replug
 them before they will work with an XO1.

 The easiest way to "trip" them is to plug them into an XO1.5 with a
 battery that needs charging.  In the untripped state, they will often
 successfully drive a battery-less XO1.5.  But once tripped, they won't
 drive the battery-less board until you reset them by unplugging and
 letting them rest.

 It is possible that the bad ADP-15 PH A adapters are samples that we
 eventually rejected.  They say "NOT FOR SALE".  The rated output current
 is 1.25A, whereas the good adapters are rated at about 1.4 A.

 I have consigned my ADP-15 PH A adapters to the bone pile.
