 good catch.  possibly related.  unfortunately there are no logs attached
 to #4476, so we can't see for sure.  the important line from this bug's
 dmesg is:

 [1147916.862327] ehci_hcd 0000:00:0f.5: HC died; cleaning up
 }}}

 i'd be surprised if none of the people that looked at #4476 noticed that.

 next time i see the problem (it's happened a couple more times since
 filing this bug) i'll take more time and see if all of USB is dead.
