 It seems firmware failed to finish AD_HOC_START (0x2b) command. Is this
 easily reproducible with a particular build that we can try out here? Also
 does this happen more frequently with firmware 5.110.22.p16 onwards? We
 did a couple of changes to meet memory requirement to support active
 antenna upgrade on regular firmware and these problem may be because of
 that memory restructuring. However, I never noticed any such failure so
 far (thanks for the update).

 Can someone try with the attached debug firmware version 0.0.0.p20
 http://dev.laptop.org/attachment/ticket/8666/usb8388_0_0_0_p20.tar.gz and
 post the dmesg log? It will help us to nail down the exact problem. Also,
 I have included a possible fix assuming that lack of memory is problem
 here, to confirm our findings so far (identified by the events received
 from the firmware on the host).
