 It's raised by the host controller.  From #2621:

 > The command is never received because the host controller reports a
 transaction error. According to the EHCI Spec the only causes for
 transaction errors are CRC, PID (Packet ID) or Timeout errors.

 This isn't the scheduling bug that I saw last night, so we're in bad
 shape.
