 Device: Kingston 2GB Mobility Kit - USB to MicroSD adapter using "Silicon
 Motion SMI331" controller, also known as "Transcend TS2GUSD-S3" USB /
 MicroSD reader.

 Symptom: OFW fails to read data from (the MicroSD card plugged into) this
 device.  Observed symptoms vary from hangs, to divide-by-zero errors, to
 complaints of too-large transfer size.

 The root cause of the problem is that this device initially doesn't return
 any data in response to the "Read Capacity" command, despite the fact that
 it reports "success" in the Command Status Wrapper protocol phase.

 Retrying the command doesn't help; to make it work, you have to either try
 to read a block of data or perform a "Request Sense" command, both of
 which also fail to return any data, but thereafter subsequent commands of
 all types work normally.
