#3630: Need fullscreen mode

We've recently decided to add fullscreen support across the board for
(almost?) all activities. Read is a prime candidate, since
scrolling/paging can happen easily without the toolbar.

The fullscreen icon should appear inside the view toolbar, at the far
right. When it is pressed, the toolbar should hide providing a fullscreen
view of the content, with one onscreen floating button (in the upper
right) for returning to standard view (bound to esc). We'll want a
simple way to toggle between full/standard view as well, but I'm not sure
what that is yet. I'd suggest ctrl-F (fullscreen), but that's often used
for find as well. How about ctrl-M (maximize) -- this kind of works since
on OSX at least command-M is minimize, which while, opposite is a
 window/view command.

As a bonus, the enterFullscreen method can also be called when entering
handheld mode, since we definitely don't want the toolbar in the way
there. Since these features go hand in had, and supporting handheld mode
sanely for at least a few of the core activities seems important, I'm
making this a high priority item.
