 Right now the Read toolbar contains forward/back buttons and a small entry
 field for specifying the page.  These forward/back buttons actually act as
 previous/next page buttons, when in fact the semantics are much different,
 and in fact we may need both sets.

 The current next/previous buttons should be replaced with the icons I will
 attach to this ticket. If, as I assume, we support internal hyperlinks
 then we'll also need the back/forward buttons for jumping through the link
 history.
