 Ok, not the (known) CAFÉ problem then -- this really seems to be the
 flash. Does it still happen if you set the three timing registers of the
 CAFÉ to 0xFFFFFFFF to use the least aggressive timing possible?

 While in QSMC last month, I believe I saw one flash chip which would
 exhibit symptoms like this, but only in the range 0x10000000-0x1fffffff of
 the flash. I believe it was in the machine with MAC address
 00:17:c4:03:55:ad (cf. https://dev.laptop.org/ticket/1905#comment:41 )
