 With extra output in the URB completion callback:

 At 9322.295 we send the last successful command; a scan. At 9322.511 we
 get the response.

 At 9323.180 we send another scan command. The URB transmission completes
 successfully, but we never get a response to the command.

 At 9323.841,  9324.840 and 9324.898 we submit TX packets, and the URB
 transmission is fine.

 At 9325.837 we submit another TX packet, and the URB transmission never
 completes. We never again see a successful TX URB transmission.

 Beginning to suspect the firmware now; would like to see this with a USB
 analyser.
