neighboring button

 It's quite common (for me) to have the secondary button palette be
 switched to a neighboring one, when traveling the cursor from the button
 to inside the palette.

 Try this in Paint:
  1. Hover the pencil button in Paint.
  2. Move the cursor to size arrows. There's a good chance the cursor will
 pass over the Brush button, switching the palette.

 This always aggravates me initially, and even after forcing myself to be
 careful when handling with the palettes (which shouldn't be necessary), it
 keeps happening.
