 Somewhat related to #1802, since I'm getting the same error message, and
 because it seems to trigger in a similar way.

 OFW: Q2C18
 Build: 485

 Sometimes, depending on how the USB stick is inserted/removed (usually
 removed), the XO may become unable to mount USB sticks until it is
 rebooted. Steps to reproduce:

 1. Insert a USB stick and mount it ("mount /dev/sda1 /mnt")

 2. Unmount it ("umount /mnt")

 3. Remove the USB stick slowly from the USB slot. Wiggling it around
 slightly seems to trigger the bug most often.

 4. You will get the following error message (many times:)
 "FAT: FAT read failed (blocknr 244)

 scsi 0:0:0:0: rejecting I/O to dead device"

 At the end you'll get this message:
 "FAT: unable to read inode  block for updating (i_pos 10423)

 5. Reinsert the USB stick and try to mount it. You will get the error
 essage "mount: you must specify the filesystem type." This will happen
 whenever you try to mount a USB stick, no matter what, until you reboot.
