 Please supply more details. Logs would be especially useful.

 I'm aware of one case where this appears to happen - if an icon gets stuck
 on the mesh view when everybody leaves (e.g. #2616), when you attempt to
 join you seem to get a new instance of the activity instead of a shared
 one. Could this have happened? If so, it's a different problem.
