 Reproduced the problem on a B4 with JTAG.  With the debugger we could see
 that the firmware is in a valid state waiting to receive a command on ep2
 to resume USB communication.  The command is never received because the
 host controller reports a transaction error.  According to the
 [http://www.intel.com/technology/usb/download/ehci-r10.pdf EHCI Spec] the
 only causes for transaction errors are CRC, PID (Packet ID) or Timeout
 errors.
