 Further analysis with Russ on IRC shows that the driver thinks the
 device's block size is 0.  The device rejects the "read capacity" command,
 and responds to the "mode sense" command with a nonsense value that
 includes the USBC envelope in the buffer.  The buffer contains 55 53 42 43
 40 00 00 00 0c 00 00 00 ff ff ff ff

 The SD card in question is a SanDisk 1.0 G with no further identifying
 markings.  That SD card works fine in the XO's SD slot.

 A 32MB Compact Flash card in the same multi-reader works under OFW.

 Russ is loaning me the failing reader/card combination for further
 analysis.
