 David Woodhouse suggested that this would happen after warm reboots.

Comment:

 This may have been fixed in version 5.220.9.p3 of the firmware.  Warm
 reboots now reset the usb8388 and revert control to the bootloader, who
 will report the correct version.  Ronak has tested this on a test-B board,
 but I'll leave this open until someone else can confirm the fix.
