#367: Robust colormap handling

Right now, the colormap is set up once by the firmware,
and the X server messes with it to get better video playback.

If it were to crash, it would leave the colormap in an incorrect state.

And to implement direct color, robust colormap handling is a must.

Arguably, the device driver should deal with colormap loading; it has to
know how to ensure the console always works on VT switches or if X
crashes. And this would let us hide DCON from X in a robust way.

We didn't have time to do this for BTest1.
